Blood in Urine, Feeling Dizziness, Creatinine 1.5

Q: I had blood in my urine about 7 months ago. later I consulted some doctors and they told me to do ultrasound which I did and was normal. later after some months i feel like dizziness and feel difficulty in doing my daily works. Then I had my urine test and blood test.

My urine test was fine but creatinine in my blood was found to be 1.5 and doctors told me to do it again but the level was found to be same. Doctors here are trying to figure out the possible cause of rise in creatinine. I am a bit confused wat is the problem with me? I attach my medical report, please check it.
