Severe Shoulder Pain in Dialysis PatientsSevere shoulder pain is common for patients on long-term dialysis. In some cases, dialysis patients’ shoulder pain becomes more serious at night that makes them fall or keep asleep difficultly. Dialysis is one treatment that can replace diseased kidneys to filter blood but fails to maintain electrolyte balance, secrete EPO and eliminate large molecular substances like beta-2-microglobulin. Just because this, dialysis can cause shoulder pain through two aspects.

One is calcium pyrophosphate (CPP) deposition. CPP is one substance formed in the cartilages of joints. Dialysis, both hemodialysis and peritoneal dialysis, can lead this substance to be deposited as crystals on joint tissues. This is called calcium pyrophosphate deposition (CPPD), and it can cause the same symptoms as gout. Along with knees, hips and wrists, shoulders are often affected, resulting in shoulder pain.

The other is beta-2-microglobulin amyloidosis. Because this substance is large molecule, dialysis fails to eliminate it from the body efficiently. This can also induce shoulder pain easily.

Based on the above causes, some treatments that can remove the above substances are helpful to ease dialysis patients’ shoulder pain.

1. Plasma Exchange: It is one blood purification method that can help exchange polluted plasma with healthy plasma substitute, so waste products can be removed from the body.

2. Blood Purification: It uses various methods including dialysis, hemofiltration, plasma exchange, immune adsorption, etc, to cleanse the body. Therefore, CPPD and beta-2-microglobulin amyloidosis are eliminated.

3. Clear Blood Pollution Therapy: Compared with the above methods, it focuses on cleansing the polluted blood and then repairing damaged kidney cells. After achieving the restoration of impaired kidney cells, kidneys can discharge waste products effectively, so shoulder pain is eased successfully.
