Symptoms of Stage 4 CKD

Chronic kidney disease (CKD) can be divided into five stages based on glomerular filtration rate (GFR). In stage 4 CKD, the kidneys are damaged severely with a severe decrease in the GFR to 15~30 ml/min. as kidney function declines, high levels of wastes build up in body, thus resulting in a myriad of symptoms. The symptoms of stage 4 CKD are as follows:


Fatigue is commonly seen in symptoms of stage 4 CKD. The patients can experience extremely tired. It can affect the patients’ life seriously. Many patients quit their job due to persistent fatigue and loss of energy. Anemia is the primary cause of fatigue in CKD.

Retention of fluids

As the kidneys are less efficient to control how much fluid stays in body, it can cause retention of fluids in body. A person can notice swelling in their lower legs, hands or face around the eyes. With too much fluid, someone could feel shortness of breath.

Urination changes

If there is loss of proteins in urine, the urine will become foamy. Red, color-colored urine can occur if it contains red blood cells. A person may urinate more or less than normal, or have frequent urination during nighttime.

Nausea and vomiting

Nausea and vomiting are commonly seen in symptoms of stage 4 CKD. As kidneys fail to eliminate toxins from body, urea is decomposed into ammonia in intestinal tract to stimulate mucosa in gastrointestinal tract, leading to nausea and vomiting. These conditions become particularly apparent early in the morning.

Sleep problems

Some people with CKD have serious sleep problems. Skin itching, muscle cramps, shortness of breath or restless legs can make them awake all night long.
