Chronic Kidney Disease (CKD) and Thyroid

Chronic kidney disease patients, compared with other people, is at higher risk to develop thyroid disorder. The common examples are hyperthyroidism and hypothyroidism.

The interaction between kidneys and thyroid functions are known by us for years. On one hand, it is vital to the balance of water and electrolytes, on the other hand, our kidneys took part in the metabolism and elimination of thyroid hormones (TH). Not only the thyroid problem affects kidney function, but also damaged kidneys pose influences on thyroid and its hormones.

I list two major changes between TH and kidneys:

Nephrotic syndrome and thyroid function changes

Nephrotic Syndrome is closely associated with serum TH levels. Due to the massive proteinuria leaked from urine, the TH changes accordingly, results in pale face, systemic swelling, chilly, fatigue, rustiness, elevated blood lipids and basal metabolic rate etc.

TH reduces according with the occurrence of Nephrotic Syndrome, as reduces to a low level, clinical symptoms would occur, but the serum thyroid stimulating hormone (TSH) stays normal. This indicates that the lowered TH induced by glomerulopathy is because of the kidney disease, rather than the pathological changes of pituitary thyroid axis.

Chronic Renal Failure and thyroid

The Chronic Renal Failure (CRF) affects greatly with the thyroid, including secretion, metabolism and excretion etc. according to the lab test results, patients with Chronic Renal Failure obtain pale face, dry skin, swelling, chilly, weakness, hypersomnia, constipation, reduced tendon reflex and time of reflection interval prolonged etc. all those are associated with hypothyroidism. Many experts are believed that this is a kind of spontaneous normal protective reaction from human body and aims at slowing the progression of Chronic Renal Failure.

Dialysis clears the urea nitrogen and creatinine out of human body, but also involved in the changes of thyroid hormones, which indicates that more complicated factors exist. However, the thyroid function promotes after performing kidney transplantation, serum T4, T3 increases to a normal level, TGB elevates accordingly too.
