Hypertension 10 Years, BUN 120 and Creatinine 15

Details: I am female age 16 and have hypertension since 10 years. Now, my blood urea nitrogen is 120 and serum creatinine is 15 mg/dl. I want to know what stage kidney disease I am on and how to manage my problems.
