Stages Of IgA Nephropathy

Stages Of IgA NephropathyIgA is divided into five stages according to its pathologic change, which is put forward by a Korean scholar who has the surname of LEE, so this method of classification is also called LEE classification.

IgA is a common kidney disease, and according to the IgA Nephropathy Support Network, in united, IgA is the third leading cause of chronic kidney disease, and it is after diabetes and hypertension.

Stage one IgA: in this stage, most glomeruli in patients’ kidneys are normal, and the kidney tubules do not have any change, besides, the renal interstitium is also normal in this time. While there may be broadening and cell proliferation in mesangium in this time.

Stage two IgA: in this stage, there is no change in patients’ kidney tubules and renal interstitium, but in the mesangium, proliferation and sclerosis often appear in this time, and the cells in mesangium can form crescent, but it is rare to see.

Stage three IgA: in this stage, the proliferation and sclerosis in mesangium will be expanded, and the diseased area is more than 50% in microscopy, which is called diffusibility lesion. Crescent may be seen occasionally in this time. Edema can be seen in renal interstitium, and it is rare to see renal tubular atrophy.

Stage four IgA: there are severe proliferation and sclerosis in mesangium, and there are partial or whole sclerosis in glomeruli. Crescent is easy to be seen in this time, and it is less than 45% in microscopy. Besides, renal tubular atrophy will occur, and foam cells occasionally appear in this stage.

Stage five IgA: in this stage, the lesion condition is like stage four, but it is more serious, the crescent can be seen in microscopy is more than 45% in this stage. The lesion in kidney tubules and renal insterstitium is also more serious than they are in stage four.

The is the five stages of IgA nephropathy. This classification is objective and comprehensive, which can reflect patients’ kidney condition clearly and accurately, so this method is accepted by the world widely.
